| Demographic Declaration |
This document should be reviewed, signed and returned to SATAI either by mail or fax. This document is entirely voluntary but does assist SATAI and the South Texas RCIC in its reporting requirements to various federal, state, and local governmental agencies. |

This document should be reviewed, signed and returned to SATAI either by mail or fax. It is a description of the respective responsibilities of SATAI, the South Texas RCIC and ETF applicants.
